Report: Worker who fell from Canada Place sails not anchored down

Worksafe BC has released its report into the death a man who fell while working on the iconic sails at Canada Place a year-and-a-half ago.

It was the second death at a Ledcor site on December 2nd, 2010.

Diego Eduardo Herrera’s company was working on the Canada Place sail rehabilitation project – when the Texas native fell 14 meters.

He later died at Vancouver General Hospital.

According to Worksafe, Herrera was wearing a fall protection harness, but died because he was not actually tied off to any of the available anchor points.

The report also finds two underlying factors in Herrera’s death – he deviated from his work plan, and on a wet and frosty morning, the Teflon-coated fabric sail roof was extremely slippery.
 
The report into the other death at a Ledcor site that day has yet to be released.

28-year-old Dan Martens from Mission was killed while working on a luxury condo development in Coal Harbour.
