Two companies could face fines after a construction worker was killed in 2010 in Vancouver’s Coal Harbour neighbourhood. Worksafe BC has released its report into the death of 28-year-old Dan Martens.
The Mission man was killed when he was hit by a concrete wall form while working at a 32-storey condo complex.
The agency says it’s considering penalties against HP Construction and engineering firm Ted Newell Associates Inc.
Factors that contributed to the death include an unsafe procedure to climb wall forms.
A report says HP and Lower Mainland Steel had safe protocols in place, but workers paid no heed. It adds that Ted Newell Associates drafted drawings for the ground floor, and not for different work being done on other floors.
